Laser Safety Laser Registration Procedures

Welcome to Environmental Health and Safety Laser registration. Registration is required for all Class 3B and Class 4 lasers, and Class 1 with embedded Class 3B or Class 4 lasers. To complete the laser registration, please follow the steps below.

1. Download the fillable PSU Laser Registration form.
2. Gather the required PI, Alternative contact and Laser information.
3. Fill out the registration form.
4. Email a copy to Yuanqing Guo.
5. Print a copy for your record.
