Report a Safety Issue
Penn State is committed to protecting the health and safety of our employees, students, visitors, and the environment. The entire University community plays an important role in achieving this objective. We welcome questions and recommendations to improve safety and health conditions or reports of potential hazards University-wide.
All members of the Penn State Community with an active Penn State Account are encouraged to report safety issues or concerns to EHS through the LionSafe system.
Do not use LionSafe to report accidents, injuries, or life-threatening emergencies. Call 911 for any life-threatening emergencies such as fires, explosions, serious injuries, and any other incident which poses an immediate threat to life or the environment.
To report a safety issue or concern, log in to LionSafe.
After logging in, select Request/Report, then New Request/Report.
- Choose General Request for general safety issues or concerns, then complete the form.
- Choose Report Event for EHS-related incidents and near mises, then complete the form.
