The purpose of this program is to establish procedures to protect employees from injury during the servicing of equipment by preventing unexpected energization.

Program Applicability

This program applies to:

  • University employees and contractors who service, maintain, or modify equipment where the unexpected release of hazardous energy could occur.
  • Lockout Tagout (LOTO) is used to prevent the release of hazardous energy, including electrical, hydraulic, pneumatic, and mechanical energy, during equipment servicing, maintenance, or modification activities.

Exclusions

  • Work on cord‑ and plug‑connected equipment is not covered by this program when unplugging the equipment controls all energy and the plug remains under the continuous control of the employee performing the service work.

Program Requirements

Supervisor Responsibilities

  • Be knowledgeable about the Lockout Tagout Program and how it applies within their areas of responsibility.
  • Identify employees who service equipment and ensure they receive appropriate LOTO training.
  • Ensure employees comply with all provisions of the LOTO program.
  • Ensure employees are provided with adequate locks, tags, and lockout devices.
  • Ensure equipment‑specific LOTO procedures are developed and implemented.

Employee Responsibilities

  • Comply with all requirements of the Lockout Tagout program.
  • Attend required LOTO training sessions.
  • Promptly report any LOTO‑related concerns to their immediate supervisor.

Training Requirements

  • Lockout/Tagout training must be provided to all employees who service, maintain, or modify equipment.
  • Supervisors are responsible for providing training on equipment‑specific LOTO procedures within their work units.
  • Retraining is required when:
    • LOTO procedures, job assignments, processes, or equipment change and create new hazards.
    • Employees do not demonstrate adequate LOTO knowledge during annual inspections.

Self‑Inspection Requirements

  • Supervisors or Safety Officers of employees trained in LOTO must complete an annual inspection to verify adherence to LOTO procedures.
  • Inspections must be conducted using the LOTO evaluation checklist available online.

Recordkeeping Requirements: Supervisors or Safety Officers must maintain

  • A list of all authorized employees trained in LOTO.
  • Copies of:
    • Equipment‑specific LOTO procedures
    • Lock removal forms
    • Contractor notification forms
    • Annual inspection forms
